OHS GENERAL AUDIT

Mining inspection is the visual inspections performed by


professional miners and safety officers during safety checks.
General audit was done to ensure the protection of life and
prevention of injuries in the mine, engineering and compliances
with regulatory requirements, MSHA standards.

It was done by SHEC team and OHS consultant G. Ferreira.


It started by giving him general overview of how SHEC is
performing at Rutongo mines health and safety,by having
contribution in community around the company.
After inspection he advised according to the departments.

SHEC DEPARTMENT
Every safety officer must have technical skills and practical knowledge about how things work in
underground.

About investigation reporting,

Investigation
Investigation report must be clear by including all required/relevant information
Clear incident description
Before root causes we have to look at immediate root causes first.
Corrective actions must be the facts to be implemented.
Witness statements and their signatures must be included. ask them separately.

Investigation team
Investigation team must have person who is responsible/head of department must
take that responsibilities.

Safety officer must be a part of investigation team as a facilitator by making sure


that corrective action taken are safety concerns.

Safety team must be involved in management team, planning team, decision


making team so that everything can be done safely by setting safely procedures
by also having a general view of everything from external aspect.

If the system/management is to be changed, we must play a bid role by sharing


impacts of those changes, and see if it will be safety improved.

We have to be reliable at the same time .

Blasting operation

Near explosive magazine must be a box that could be used to store (keep)all
electronic devices (headlamps, telephones, batteries….) so that no one is
allowed to be having the explosive store even though it is at low risk. For
hand protection: blasters must be using soft rubber gloves/surgical gloves
when packing ANFO in small plastic bags.

Tramming

Tramming crews must be trained frequently about tramming procedures.


When there is a moving machine, people must be 5 meters away .

Welding team

they must wear lead apron.


uplifting safety shoes for protecting them from welding sparks.
they must wear welding helmet and make inspection beforethey put them
on.

Engineering

Internal inspection of heavy equipment's like compressors must be carried out. Must be done
every six months to comply with the legal requirements.

Unqualified people are not allowed to do such inspection.

All machines that are not working must be disconnected to electricity.

On the switches must be isolation lock system (Test and Touch) by avoiding any accident that
may be caused by unqualified/untrained people using it in anyhow.

For movable machines like excavators there must be something else to support it if it is not in
motion.
Flash back must be implemented for protecting gas bottle when falling down or flames may be
backward in flexible pipes connects to gas bottle.

Grease used in lubricant must be environmental friendly because when it is low quality it can
cause skin cancel.

Advise:
 Procurement must order materials depending on safety measures (E.g. environmental
friendly equipments).
 Gas detectors must be calibrated by the company.
 Setting signboards everywhere it is necessary to.
 Covering all moving machine’s motors at main dam.

For fire extinguishers


They must be set at height of 1.5 m from the ground and set sign boards.

Upgrading plant

All movable machines such as crushers, separators…their motors must


be well covered/protected/guarded.
Fence water dam by rail fence or any strong protection.
Lifting up the roof at some suitable height.
Extraction of sucking system for sucking out dust.

VFL TRAININGS

This is a visible felt leadership training started on April 19 th and


27th, 2023 where 44 people for RUTONGO Mine safety
superintendent, safety officers, supervisors, team leaders, HR,
Security, cooperatives and subcontractors trained.
They were 1st and 2nd intakes where they trained on the meaning
of VFL, its steps and the reasons why it is important to our
company in the journey to zero harm.

The other intakes will be continuing for the rest.

BARRING AND TRAMMING TRAININGS

From 11th April,2023 safety team is training people about practical


barring procedures and About Tramming in terms of refreshment
training as a great achievement in order to make sure that people
are well trained not only verbally but practically too. The training
will be ongoing to the rest, so that every worker in the mine will
be trained.
However it is not about barring procedures And Tramming only,
they will be trained about all mining aspects.
Total number of trained workers in this week,
 Barring are 275
 Tramming are 344

OHS COMMITTEE MEETING

On 13th April,2023 Rutongo OHS committee


meeting held in board room where it started by
reviewing the last meeting where the main focus
was to arrange store of PPEs and print out safe
declaration , hazard identification and stop and fix
books.
Next meeting is planned to take place on 13rd
june,2023.
