Type of hazard: B

Description
On 21st June,2023 at Gasambya site BV20, there is B hazard identified,
where some ball valve for air pipes were not working (there had no control of
air delivered from compressor to different points of use). For instance, if there
could be an incident of air pipe burst and there is no valve nearby so that
they can be used to shut down or in any emergency case.

Recommendation
 Replace some old valve with new ones

INCIDENTS, NEAR MISS DESCRIPTION

TIME: 10:05 a.m.
INJURED PERSON: HAKORIMANA Emmanuel
PLACE: GASAMBYA site BV 20
TYPE OF ACCIDENT: T&R

DESCRIPTION
At Gasambya site BV 20 around 10:05 a.m. a subcontractor employee named HAKORIMANA Emmanuel he got injured on his right arm, when
He was trying to remove the production that was left on the face of raise with a hammer and chisel then a small piece of stone fell and injured
him on the right arm due to not barring down properly. He was wearing overall improperly since the overall arms were folded. He was given first
aid and return to work.
Root cause
• Improper barring rock down caused the small piece of stone to fall and injured him.
• Wearing overall improperly.
Recommendation
 To barring down effectively to avoid further accidents.
 To wear overall properly.

TYPE OF ACCIDENT: T&R

DESCRIPTION
At NYAMYUMBA site BV 31, west drive F10 right side of the new Raise, Around 9:00 a.m. RML employee called MUGIRANEZA
Joseph (RDO ) got injured. He was barring down and using short chisel, therefore a small piece of stone hit him on left arm
hence an accident. We provided him first aid and got back to work.
Root cause
• Barring down using short chisel.
Recommendation
 To use the adequate pinch bars regarding to where it is going to be used.

TIME: 11:00 a.m.
INJURED PERSON:TWAGIRIMANA J. Baptiste
PLACE: NYAMYUMBA site BV 31
TYPE OF ACCIDENT: T&R

DESCRIPTION
At NYAMYUMBA site BV 31, main drive around 11:00 a.m. RML employee named TWAGIRIMANA J. Baptiste ( loco driver) got injured on his finger when he was
connecting the wagon but we provide him first aid and sent him to MASORO health center.

Root cause
• Unsafe behavior (Putting your partner at risk)
This accident was caused by unsafe behavior due to the loco operator named TWAGIRIMANA Jean Baptiste, who decided to let his assistant named
HABIYAREMYE Gaspard to drive the locomotive number 008 and the loco operator became an assistant which means they switched work without mining
supervisor approval , While the local operator was trying to connect the wagons which was not in line of his job , his assistant drove the locomotive and injured
loco operators finger hence the accident.

Recommendation
Every employee must do the job he/she has been trained for.

TIME: 3:30 a.m. (night shift)
INJURED PERSON: HAMEDIGANDI Elisse
PLACE: KARAMBO M2
TYPE OF ACCIDENT: T&R

DESCRIPTION
At Karambo site M2 at 3:30 a.m. (night shift) a mine assistant called HAMEDIGANDI Elisse got injury on his left hand. When they were pushing
the wagon at a high speed and they hit another wagon that were left in the rail then the pin immediately opened and caused the wagon to
loose the balance and hit him on hand hence the accident . He was given first aid and returned to work.
Root cause
• Pushing wagon at high speed
• Not checking the obstacles in the railways before.
Recommendation
 Lower the speed while pushing wagons.
 To remove all obstacles in the railways.

TIME: 3:45 a.m. (night shift)
INJURED PERSON: HABAMAHORO Martin
PLACE: MAHAZA Site BV 71
TYPE OF ACCIDENT: T&R

DESCRIPTION
At Mahaza site BV 71 Main drive around 3:45 a.m. (night shift) a Cooperative employee named HABAMAHORO Martin when he was busy barring alone, a
piece of schist fell down and rolled over on tailings because he didn't have enough space to escape it ended up hitting him to his right leg. he received first
aid and returned to work.
Root cause
• He didn’t check if there is enough space to stand
• Not following barring procedures (he was barring alone) and yet he was supposed to have three co workers beside him for the sake of awareness.

Recommendation
 To check if there is enough space to stand while barring.
 To follow all steps of barring down (no one is allowed to bar down alone while they must be three so that other two can watch over him if he is standing
properly and looking other rock cracks to be barred down).

INJURED PERSON: MUHIRE Vedaste
PLACE: Gasambya site BV20
TYPE OF ACCIDENT: T&R

DESCRIPTION
On 26th June,2023 at Gasambya site BV20 around 12:05 PM RML employee named MUHIRE Vedaste (Timberman) he got
injury on his mouth when his assistant was trying to fix tree in the raise with hammer and it hits him due to lack of awareness
hence the accident. He was given first aid and taken to Murambi health center.

Recommendation
Before using a hammer or chisel, it is a MUST to check that no one is around/nearby to avoid any type of injury.

Date: 21th & 26th June,2023

On 21th & 26th June 2023 at Nyamyumba site, 14 (10 mine assistants and 4 loco assistants) NGIRU subcontractor
employees were conducted Health and safety induction about:
 Trinity OHS policy
 Rutongo mines safety standards
 Drugs and alcohol abuse
 Hazard identification
 Risk control
 Health and safety rules in underground.
WEEKLY INFORMATION AND POINT TO DISCUSS

 The refreshment trainings of practical barring procedures are needed so that will be given to baring
crews starting by Nyamyumba site in this week. To remind all workers to follow all barring procedure
to avoid further accidents.
 All unworking places especially the raises must be well closed.

There are some conclusions taken to be implemented regarding to the


accidents happened during this last week

According to incident happened on BV31 due to locomotive drive


switched his duty with his Assistant
• Johan said “everyone have to perform the work he /she trained for.”
• OHS Supervisor Kevin said that “if there's any inconvenience, shift
boss have to inform a safety officer what changes made so that new
workers appointed can get trainings to prevent different kind of
injuries.
