High-drop areas:
Shafts are the areas of high drop/rainy working places. So that the people working there get very wet when they are working.

• Rain coats not ponchos. Required rain coats are of suitable for the boil makers because when they are welding, the ponchos got
burned by welding sparks very easily.

INCIDENTS, NEAR MISS DESCRIPTION
INCIDENT DESCRIPTION
On 3rd June 2023 around 12:40 PM at NYAMYUMBA BV 28( East drive down dip 1 F14) a subcontractor employee named
UWITONZE Jean Claude got injury when they were cleaning by using old bags then a stone came from the bag and hit his
back.
On 1st & 5th June,2023 at Masoro site and Nyamyumba site BV 16, 6 new
cooperative employees and 4 subcontractor employees were conducted health and
safety induction about:
Trinity OHS policy
Rutongo mines safety standards
Drugs and alcohol abuse
Hazard identification
Risk control
Health and safety rules for mining.
TRAINING
On June 2nd and 3rd, 2023, blasters and team leaders were trained on the use of
explosives and Development marking but the Mine manager reminded the team leaders
about their daily duties and responsibilities regarding safety.

3. Watering down to allay dust and expose cracks in the hang and sidewalls
4. Inspect for any misfires
5. Fix any Hazards
6. Fill in your safe declaration book and let the crew sign that the working area was declared safe for work.
During shift the Shift Supervisor must over inspect the safe declaration book.
Check and inspect any Checklists that should be filled in your area of responsibility.
Look at all safety equipment and declare it safe for use in your area. .
Mid shift barring should be done during the shift. .
Any left over explosives should be removed from underground in the approved containers and sent back to the magazines.
• Ensure after the shift all people has signed out.
• Blast on blasting time.

• Continue Training about Tramming and barring in underground for all workers of
RUTONGO mines.
• Induction for all new employees and all guests.
