SAFETY IMPROVEMENT

According to last meeting, there are corrective actions and tasks which have been implemented such as:

 Jack rails/tongs for lifting and carrying rails to material guards


• Karambo, Masoro, Mahaza, Nyamyumba site BV 4, BV16.

Other sites do not have them but they made requisition for it.

 Checklists for bobcats and locomotives are available.

 Signboards for speed

May,2023 . There is accident
happened, when cooperative
employee was cleaning in main
drive, illegal miner who was
working above the old ore pass,
pushed a piece of stone and hit her
back. We recommended to close
the ore pass, it has been fixed.

were piled on top of a set support
and it was overloaded and started
to fracture (crack). Now it is fixed

have strengthened the roof
support by adding set support
timber next to damaged one. As it
is revealed it has been fixed.

 High-drop areas:

Shafts are the areas of high drop/rainy working places. So that the people working there get very wet when they are working.

Recommendations

• Rain coats not ponchos. Required rain coats are of suitable for the boil makers because when they are welding, the ponchos got
burned by welding sparks very easily.

 In charge: Management team and safety team

 Fumes:

During the welding process, there is a fog caused by welding fumes.

Recommendations

• Exhausting fans/Sucking fans.

INCIDENT DESCRIPTION
On 3rd June 2023 around 12:40 PM at NYAMYUMBA BV 28( East drive down dip 1 F14) a subcontractor employee named
UWITONZE Jean Claude got injury when they were cleaning by using old bags then a stone came from the bag and hit his
back.

Immediate action taken

He was given first aid and sent to Masoro health center.

The old bags were requested to be replaced immediately.

SAFETY INDUCTION

WORKING PLACE : Masoro site and Nyamyumba BV16


Date: 1st & 5th June,2023

On 1st & 5th June,2023 at Masoro site and Nyamyumba site BV 16, 6 new
cooperative employees and 4 subcontractor employees were conducted health and
safety induction about:
 Trinity OHS policy
 Rutongo mines safety standards
 Drugs and alcohol abuse
 Hazard identification
 Risk control
 Health and safety rules for mining.
TRAINING

DATE: 2nd & 3rd June 2023


LOCATION: Nyamyumba BV 33 and Karambo site

Training for the team leaders and blasters

On June 2nd and 3rd, 2023, blasters and team leaders were trained on the use of
explosives and Development marking but the Mine manager reminded the team leaders
about their daily duties and responsibilities regarding safety.

-Hold a Safety Meeting at the beginning of the shift


-Look at the attendance of all employees under your supervision
-Plan the daily tasks and give proper work instructions to employees
-Ensure that you order the correct amount of explosives
-Enter and do proper early morning entry examinations with all your crews.
TRAINING

1. Barring

3. Watering down to allay dust and expose cracks in the hang and sidewalls
4. Inspect for any misfires
5. Fix any Hazards
6. Fill in your safe declaration book and let the crew sign that the working area was declared safe for work.
During shift the Shift Supervisor must over inspect the safe declaration book.
Check and inspect any Checklists that should be filled in your area of responsibility.
Look at all safety equipment and declare it safe for use in your area. .
Mid shift barring should be done during the shift. .
Any left over explosives should be removed from underground in the approved containers and sent back to the magazines.
• Ensure after the shift all people has signed out.
• Blast on blasting time.

Areas for improvement


MINING
• Unsharpened pinch bars, they can not bar down properly due to lack of
sharpness.
Recommendation
• Anvil and charcoal so that a blacksmith on the site can forge them.

SIGNBOARDS FOR SPEEDING

• Signboards of 10 Km/h were posted, the remaining of 30 km/h. The


deadline to be posted on 6th June,2023.

 Engineering department is requested to help out on this.


SAFETY ACTION PLAN PROGRAM

• Continue Training about Tramming and barring in underground for all workers of
RUTONGO mines.
• Induction for all new employees and all guests.
