SAFETY INDUCTION

Location: Nyamyumba, Masoro and Gasambya sites

June 2023, 5 new Masoro cooperative employee,2 new processing plant employees, 59 NGIRU, 145 MINANI, 30
GASIGWA subcontractor employees were conducted Health and safety induction about:
 Trinity OHS policy
 Rutongo mines safety standards
 Drugs and alcohol abuse
 Hazard identification
 Risk control
 Health and safety rules in underground.
SAFETY TRAININGS
DATE: 2nd & 3rd June 2023
LOCATION: Nyamyumba BV 33 and Karambo site

Training for the team leaders and blasters

On June 2nd and 3rd, 2023, blasters and team leaders were trained on the use of
explosives and Development marking but the Mine manager reminded the team leaders
about their daily duties and responsibilities regarding safety.

 Hold a Safety Meeting at the beginning of the shift


 Look at the attendance of all employees under your supervision
 Plan the daily tasks and give proper work instructions to employees
 Ensure that you order the correct amount of explosives
 Enter and do proper early morning entry examinations with all your crews.

6. Fill in your safe declaration book and let the crew sign that the working area was declared safe for work.
During shift the Shift Supervisor must over inspect the safe declaration book.
Check and inspect any Checklists that should be filled in your area of responsibility.
Look at all safety equipment and declare it safe for use in your area. .
Mid shift barring should be done during the shift. .
Any left over explosives should be removed from underground in the approved containers and sent back to the magazines.
• Ensure after the shift all people has signed out.
• Blast on blasting time.

BOBCAT AND LOCOMOTIVE TRAINING


Date: 7th & 8th June,2023
Location: Karambo, Mahaza, Nyamyumba sites

Description
On 7th & 8th June,2023 at above mentioned sites there were trainings of bobcat
and locomotives offered by engineering and safety team that looked about:
Inspection, assessment and the important of doing it
While inspecting and assessing the machine, they trained and reminded to do it
carefully. Moreover, they explained the importance of checklists and serious
accidents caused by materials we had.

Special instructions of locos and bobcats


They reminded on the loco’s and bobcat safety instruction that it is not allowed to let people
pass near the loco while it is moving and when the bobcat is operating at least people must
be within 5m from the standing position.

Important of the checklists and filling them regularly


In addition, they explained why it is important to fill the bobcats and locos checklist regularly
so it can be easy to reveal any unsafe act or working while operating

Review of accidents/incidents caused by locos and bobcats and their root causes.
They also looked at the latest accidents/incidents that we have been having due to the fact
that they were not careful while operating the machine and the root causes caused them to
happen.

VFL TRAININGS
Date: 14th June,2023

Description
On 14th June,2023 at Headquarter held to leaders including mine,
store and engineering supervisors, team leaders, safety officers,
security attended the third intake VFL(visible Felt Leadership)
training which aims to achieve zero accident in the road of no harm.

PRACTICAL BARRING DOWN PROCEDURE TRAININGS


Date: 14th June,2023
Location: Nyamyumba site

Description
On 14th June,2023 at Sanzari BV30 there were practical trainings of barring down
procedures in underground offered about:
 Steps of barring down
 Responsible person (every one is responsible)
 How to identify the rock cracks by knocking the rock with pinch bar
 Adequate materials used (sharp pinch bar, hammer) in barring down
 Importance of barring down

OHS committee meeting

Date: 13th June,2023

Location: Headquarter

On 13th June,2023 Rutongo OHS committee meeting held in board room where it started by reviewing the last meeting
where there is great achievement on last action plans:

 Implementation of safe declaration books

• Hazard identification

• Stop & Fix

 Improvement of information sharing.

What must be undertaken:

 Mine Audit, must be done with scoring underground activities by safety and mining team.

 Arrangement of PPEs store must be well organized at end of this June.

 To retrain and give the refreshment trainings to all workers in all aspects.

 The main focus was to look together how to get safety reps in each crew and follow up to be made to ensure that it has
been implemented.

SAFETY INITIATIVES
SAFETY REPRESENTATIVES MEETING

On 29th June,2023 there was a safety representatives meeting held to review and discuss about
the regular performance of safety reps, their managerial roles and responsibilities.

It started by specifying who is the safety rep?


A safety representative is an employee who represents other employees in health and safety matters in the
workplace in each crew
Their roles and responsibilities are to emphasizing pre entry examination through reviewing the health &
safety measures in the workplace.
Their benefits from this:
 To have the special regular trainings to understand deep about their responsibilities.
 Improving employee involvement in safe working condition
 Helping the employer to identify potential risks
 Helping the employer to identify better safety controls
 Improving safety culture of an organization
