24th May,2023 Incident Bicycle damaged


On 24th May,2023 around 06:55 AM at  Follow the speed limit

WORKING PLACE: Headquarter

New asset protection superintendent, 4 internees and Group IT, conducted health and safety induction
about:
• Trinity OHS policy
• Rutongo mines safety standards and general policy
• Risk control
• Hazard identification
• Alcohol and drugs abuse
• Safety rules for mining and engineering
SAFETY ACHIEVEMENTS
There are many corrective actions which have been implemented
such as:

 Safe wagon connecting, the wagons are being connected in


safe way to prevent any accident that can occur due to unsafe
connecting and derailing.

 Demarcating close to the switches, as a typical warning sign


shows that the switches are near or we are closing to them.
SAFETY ACHIEVEMENTS

 Working at height to set supporting trees, it has been implemented where trees for supporting while
working in the rises and dip downs at Nyamyumba, Mahaza and Gasambya site were set.

 Barricading unworking places it was a crucial thing to wok on and it implementation was achieved
at Mahaza and Nyamyumba site

SIGNBOARDS FOR SPEEDING

• Maximum speed limit 10 Km/h. They will be set at HQ and in all areas for operations (Masoro,
Karambo, Gasambya, Mahaza, Nyamyumba, Gisanze).

• Maximum speed limit downward Nyamyumba site is 30 km/h


SAFETY ACHIEVEMENTS

Log books were implemented and submitted to be used as a handover, hazard revealed and how
they will be fixed .
Those log books have 3 categories
1. Hazard identification to be filled with mining supervisor must be on each BV.
2. Safe declaration to be filled by mining team Leader
3. Stop and fix for safety officers ,Superintendent level ,visitors and others given competent person to
fix what was revealed.

Some sites are doing great job in housekeeping(cleaning)


SAFETY INITIATIVES

Latrine:
For good sanitation and ensuring hygiene of working areas.
Some latrines are being made Mahaza, BV 28 A.

 Poor hygiene have impact on our health. Please take


care of site restrooms hygiene.
