PREVENTING INJURIES AND FATALITIES IN OUR WORKPLACE

• Injuries and/or fatalities continue to occur in our industry despite

lessons from previous events

• Experience and investigations tell us that the critical controls and

safeguards designed to prevent fatalities are sometimes not
functioning properly, not in place at all, or bypassed.

• We all make mistakes. We need to have systems that inherently

account for that and protect our workforce, by making it easier to get
it right and harder to get it wrong

• We have developed a culture of ‘Stop Work Authority’, we now need

a culture of ‘Start Work Authority’ by remaining vigilant of our work
environment and maintaining a sense of vulnerability.
