Definition

Erectile dysfunction (ED) is defined as the persistent


inability to attain and or maintain an erection
sufficient to permit satisfactory sexual
performance.
Prevalance
• ED is a highly prevalent health problem that
affects 30 million men in the USA.
• Due to the increasing life span and the high
incidence of ED in aging population, a further
increase in patients with ED can be expected.
• Worldwide, the affected population is predicted
to increase from 152 million in 1995 to 322
million in 2025.

Aetiology
The aetiology of ED may be vasculogenic,
neurogenic, anatomical, hormonal, drug-
induced and/or psychogenic.
C/P
• C/O:
ED, duration, ability to penetrate, response to
PDE5I.

•O/E:
1. general: look for signs of low androgen.
2. local: penis and testes.
Investigations
•Lab: eg Hba1c, lipid profile and HP mainly
serum androgen.

•Radiology: penile duplex “certain


indications”
TTT
• TTT of modifiable risk factors eg smoking,
DM, drugs etc

•Oral TTT: PDE5I.

•Local TTT eg ICI.

•Surgical TTT eg penile implants.
