Training objectives

1. To raise awareness about potential risks and hazards caused by fall from
height
2. To understand the key measures and practices to eliminate and /or
control these risks
Introduction
Why Focus on Fall from Height?

Fall from Height can cause FATALITY

Fatality in Nagorote Grinding station,


Nicaragua
Bryan Josué Ramírez Meza, a 24-year-old
contractor, was fatally injured after falling
from a roofi on April 25, 2023, at Nagarote
Grinding station in Nicaragua.

Bryan Josué Ramírez Meza was applying


waterproofing to the warehouse roofing. He
was walking on the roof when he stepped
on a skylight, it collapsed and he fell 9
meters.

Barriers, Railings & Covered openings

• Fixed barriers have height, strength and are


positioned to prevent a fall

• Non-fixed temporary barriers are positioned 2


meters from the edge or opening and fully
surround the hazard

• Visible warning signs are clearly placed near


the work area

Q ) Specific objectives of barriers, railings & covered openings:

Creates a physical barrier that prevents a falling from height at edges or openings with fall

Q) What are the Critical Control Performance Requirements:

a) Minimum height of 95cm to top rail, toe boards of at least 10cm, no unprotected gap
between toe board and midrail or midrail top and top rail with more than 50cm

b) Temporary barriers that are not fixed to the ground must be placed at least 2m from the
edge of the opening, are clearly visible marked with hazard signage

c) Floor opening cover capable of to withstand any potential load, is secured in place and
marked with it’s capacity

d) Handrails, guardrails, and coverings are made of rigid and resistant materials. Permanent
or fixed handrails need to withstand 1kN

Q) What is the Critical Performance trigger for shutdown, critical control review or investigation?

Physical barriers are not meeting the control performance requirements or are not in
place where a fall hazard exists

• Check if a safe alternative method is possible, if


not, work must be approved by a competent
person

• Task- specific plan is determine and known by


all involved for safe work at height and safe
rescue

• Work permit outlining the review, risk


assessment and rescue plan is authorized and

Q ) Specific objectives of barriers, railings & covered openings:

Prior evaluation of the work if above 1.8m, to determine if alternative safe methods are available, and
specific assessment of relevant hazards, appropriate controls for safe execution and rescue,
documented and authorized with a permit to work

Q) What are the Critical Control Performance Requirements:

a) Assessment of work to first determine if work can be done without work at height and, if not,
an assessment of the potential hazards and associated necessary controls

b) Development of a specific plan for rescue in the event of an emergency that has been
reviewed and authorized

c) Completion of a permit to work that validates proper development of risk assessment ,


method of work, and rescue plan for the specific task by a competent

Q) What is the critical control performance trigger for shutdown, critical control review or
investigation

Any form or document associated with the work task that is missing, incomplete, incorrect, or
not authorized and signed. Any elements of the plan that are not being followed in the execution
of the work task

Q) What are the specific objectives for:

Certified structures, platforms, and work access equipment (e.g. MEWP, scaffold) are designed, set up
and used properly when performing work at height tasks

Q) What are the critical performance requirements to meet the objectives?

a) Physical requirements including size, strength, materials, markings (tags/labels) are


adequate

b) Safe usage requirements including position, set up, distance, from edges, signages, and
speed/ area of movement

c) The inspection and maintenance requirements are done annually and before each use

d) Assembly, inspection and use of the equipment is by only those who are authorized

Q) What is the critical control performance trigger for shutdown, critical control review or
investigation?

Any equipment or part of a structure, platform, or work access system that is not fit for use OR
not authorized
Personal Fall Protection Equipment
• Only worn by persons who are trained and
authorized to inspect, wear and use

• Equipment has been inspected by user, is fit


for use and worn correctly

• Equipment is always connected to a suitable


anchor point while working at height

Q) What is the specific objective for personal fall protection equipment?

Authorized persons working within 2 meters of an unguarded opening or edge (1.8m meters or
higher) wear and use personal fall protection(restraint/arrest) attached to approved anchor point
at all times

Q) What are the critical performance requirements to meet the objective?

1) Adequate strength in systems to withstand forces applied in potential event is 22kN for anchor
and 16kN for connectors

2) Annual and pre-use inspection, testing and maintaining of personal fall protection equipment

3) Only trained and authorized persons, involved in working at height activity, supervision,
permitting or execution

4) Personal Fall Protection System is used correctly including the selection, fit, and connection
choice for bodywear, connectors and anchor Assembly, inspection and use of the equipment is
only by those who are authorized

Q) What is the critical control performance trigger for shutdown, critical control review or
investigation?

Any personal protective equipment available for use or in use that is not meeting
performance requirements and certified or persons involved in either supervision,
permitting or execution of work at height tasks that are not authorized
