ACID FAST STAINING
• Ziehl-Neelson method
• BI is a semiquantitative measure of AFB density in smears, this is useful to help determine the
type of leprosy and monitor treatment results
• IM is the percentage of leprosy bacilli in solid form to all AFB. The morphological index is
useful for determining the transmission power of germs, assessing the results of treatment and
helping determine drug resistance
Specimens
• Nasal mucosa, skin lesion, ear lobules
• Skin and neve biopsy
A. Skin
• Edges of the lesion
• Slit and scrape method :
• Pinch the site tight
• Incise
• Scrape and collect material
• Smear on a slide
B. Skin biopsy
• Active edges of the patches

C. Nerve biopsy
• From thickened nerves

• For histological conformation of tuberculoid bacilu (as the cannot be demonstrated in direct
smear)

LEPROMIN TEST

• Delayed type of hypersensitive reaction

• Lepromins used as antigens may be of human origin (lepromin H) or armadillo derived
(Lepromin A)
• Dose : 0,1 ml
Serological test
• to detect antibodies against M. leprae antigen (PGL-1) by means of the enzyme-linked
immunosorbent assay (ELISA), lateral flow test, dipstick, and hemagglutination test.
• To determine the type of leprosy
• PB leprosy, serological examination results are often negative because of low humoral
immunity
• MB leprosy, positive results
• Relapse, increased yields

• Positive for allergic contact dermatitis
• By attaching the allergen to the infected area, the test results are read 24,48,72, 96 hours
Wood lamp
• Tinea capitis : greenish yellow
• Pityriasis versicolor : golden yellow
• Vitiligo : depigmented macules

apigmentation
