THE HOME HEALTHCARE COMPANY THAT PROVIDES
YOUR VISITING NURSE

1. Evaluation
Nurses often take patient histories to look for
clues as to the causes of the current condition,
while the doctors diagnose disease and injury. X-
ray technicians, mental health professionals and
specialists often collaborate to make assessments.
Patient histories
 The patient was Mrs. Ani, a 60-year-old diabetic patient. For the past
few weeks she had been having problems with a foot wound that was
difficult to heal. The patient's family doctor recommended home
wound care as her foot condition did not allow her to travel far to the
hospital. Therefore, Mrs. Ani decided to invite a home nurse care to
her house to help with her diabetic wound care. Mrs. Ani wanted to get
the best care to speed up wound healing and also wanted to learn how
to care for wounds so that they would not get worse in the future.
 Let’s me Introduce myself My name is …. as your nurse, who will be
taking care of your wound today. Before that, I will evaluate the
wound on your leg, your previous medical history, including your
medications.
2. Treatment

Health care providers deliver explain the

procedures necessary treatment through a
variety of venues. Pharmacists fill the
medication orders given by the treating
physician who diagnoses the patient and
prescribes a treatment plan. Physical therapists
work with patients' physical rehabilitation,
while psychologists and psychiatrists treat
emotional and mental disorders.
Treatment (explain the procedures)

 Before taking action, I will first explain the procedure that

will be carried out in this wound treatment. First, I will clean
the wound and its surroundings using an antiseptic solution,
after drying and applying antibiotics, I will cover the wound
using a special bandage to protect and keep the mother's
wound moist. When the procedure is performed, it may hurt a
little because it must be cleaned properly to prevent infection
and speed up the healing process. I will start the procedure,
please cooperate.
3. Care

Short- and long-term care are provided by a

host of health care providers, from doctors and
nurses to nurses' aides and personal care
assistants. Health care providers may become
an integral part of an ailing person's life and
present for most activities. Health care
workers specializing in hospice care attend to
the needs of the dying, while those involved in
midwifery accompany women through the
childbirth process.
Care (short term and long term
services)
 I will then explain the short-term and long-term care of your
wound. For short-term care, you should avoid strenuous
activities or walking long distances to avoid putting pressure
on the foot that could worsen the wound. You should also
keep your foot and wound clean, avoid using alcohol or
cigarettes, and follow your doctor's medication schedule. For
long-term treatment, you should pay attention to a healthy and
balanced diet and diligently do physical activities such as
walking or light exercise to maintain health and improve
blood circulation in the legs.
4. Prevention

Health care providers take on the role of

educators as well as healers. Dentists train
patients on how to brush and floss daily to
prevent tooth decay and gum disease.
Physical therapists provide patients with
exercise plans and tools to avoid further
injuries. Nutritionists help patients form
healthy eating patterns and counselors give
patients an outlet for their emotional issues
to prevent breakdowns.
Prevention (giving education)
 The action has been taken, then I will tell you how to avoid more serious
injuries. Blood sugar levels are very important in the healing process, as
high blood sugar levels can slow down the wound healing process. It is
important to maintain hygiene and avoid infection, changing dressings
regularly with proper wound care. How to clean the wound as I did
earlier. For footwear selection, it is recommended to use the most
comfortable one according to you and avoid using hot water on the
wound area. After that, the symptoms to watch out for include wounds
that are difficult to heal, changes in the color and shape of the foot, pain
or tingling. You should immediately consult a doctor if you find these
signs. Is that understood ma'am?
5. Administration
Health care providers in administration make
sure that laws and guidelines are followed
and insurance policies are met to pay for
patient care. They keep important medical
records and protect patient privacy.
Administration (ensure health
insurance membership)
