reopened from November 21, 2022 until January 10, 2023.

If ever there is a discrepancy in the LIS , the Live Birth/PSA late presented by the
parent/guardian to the teachers and the housekeeping is already closed the

2.School Form 2 or SF 2-DAILY ATTENDANCE
 Must be LIS generated-if there are
trans in/out-generate again.
 Follow the codes for checking of attendance-
(blank-Present;(x)absent;Tardy(half shaded=Upper
for late Commer,lower for Cutting class)
 For the REMARKS (NLS or No longer in School-State the reason
that stated in legend #2 , If transferred OUT/IN write the Name
of School).

4.School Form 3 –Books Issued and Returned
 if the school is releasing books and other learning materials to
learners, SF3 may be used. This form can also be utilized to
record the distribution and retrieval of books. In using SF3 for the
recording and monitoring of books for property custodian
record, teacher write the title of the books and dates being
issued and returned.
 Submit SF3 after the 4th Periodical Test to Property
Custodian/School Guidance for School Record Purposes and
Property Inventory.
 SF3 must be LIS generated.
5.School Form 5 - Report on Promotion
 must be LIS generated.
 Class Adviser is accountable to check/open the
LIS account if the SF5 is available for encoding.

to Sign the School Form before the District Checking.

6.School Form 8 –Basic Health and Nutrition Profile

 parents or guardians even teachers may administer the


actual measuring of weight and height (Body Mass
Index) or request the assistance of a barangay health
worker and submit a reading result to the class adviser.

7.SF 9 / Form 138 – School Report Card

 Instruction and guidelines related to assessment and


computation of grades. The K to 12 Education Program
uses standards and a competency-based grading
system. These are found in the curriculum guides. The
minimum grade needed to pass a specific learning area
is 60, which is transmuted to 75 on the report card. The
lowest mark on the report card is 60 for Quarterly
Grades and Final Grades.

 DepEd Order No. 15, s. 2016 shall be followed in grading


kindergarten pupils’ progress. Checklists and anecdotal
records are used instead of numerical grades in both
situations. These are based on learning standards found in the
Kindergarten curriculum guide. Grades 1 to 12
 To evaluate student learning at particular points in each
quarter, summative assessments shall continue, written works
(at least 4) and performance tasks (at least 4).

Assessment and Grading in Light of the Basic Education
Learning Continuity Plan, discusses guidance on the
assessment and student learning and on the grading scheme
to be adopted this school year which is still basically anchored
on DepEd Order No. 8, s. 2015 titled Policy Guidelines on
Classroom Assessment
