(CACHECTIC, MARANTIC) ENDOCARDITIS
Non-bacterial thrombotic, Etiology-
cachectic, marantic or terminal advanced cancer (in 50% case of
endocarditis or endocarditis
NBTE) especially mucinous
simplex is an involvement of the
adenocarcinomas, chronic
heart valves by sterile thrombotic
tuberculosis, renal failure and
vegetations.
chronic sepsis.
allergy, vitamin C deficiency, deep
vein thrombosis, and endocardial
trauma (e.g. due to catheter in
pulmonary artery and
haemodynamic trauma to the
valves).
Morphology
Microscopy-the vegetations in NBTE
are composed of fibrin along with
entangled RBCs, WBCs and
platelets.
Vegetations in NBTE are sterile,
bland and do not cause tissue
destruction.
The underlying valve shows
swollen collagen, fibrinoid change
and capillary proliferation but
does not show any inflammatory
infiltrate.
