 Points to be noted
ASHA Should fill up 7-10 CBAC/WEEK .
Telemedicine Consultation Per SK must be 10 to 15 per day and Per MO consultation must
be 60 per day.
Atleast 5-6% of target pop.(above 30 yrs.) must be screened (PBS&Opportunistic) for HTN
&DM Per Month/SC-SK.
Weeklong observation of 6 health calender days (world
Hypertension ,Diabetes,Heart,Stroke,COPD &Cancer day)should be utilized for above
awareness building and camp based screening activities.
All Diabetes patients(new&follow-up) must be TB Screening.
All CHO should be entry Daily & Monthly Report in HWC portal for SC-SK and also Staff
Nurse should be entry daily & monthly report in HWC portal for PHC-SK on regular basis.
All CHO must be entry monthly service details report in SKMIS portal within end of every
Month and all block level service details data validation,edit & PBI entry completed as per
early mentioned time frame.
