ACCIDENT.
Is unintentionally or unexpected event that occurs which causes damage
of property or injury to people.

ACCIDENT INVESTIGATION.
Is the process carried out in order to determine the root cause of an
accident so as to prevent further accidents of such.
Or
Is the process of doing research on the cause of accident.
GUIDELINES OF ACCIDENT INVESTIGATION.
1.You need to have a good decision making.
2.Take immediate action.
3.Find a person of the of a particular department to prevent an accident.
4.Conduct Interview.
5.Gather evidence.
6.Take in action.
PURPOSES OF ACCIDENT INVESTIGATION.
1) Protection of adjacent running lines.
2) Protection of site of accident.
3) Save life and alleviate suffering.
4) Protect property including mails.
5) Provide succor and help to the passenger at site.
6) Transportation of stranded passenger.
7) Preservation of clues and ascertain the cause of accident.
8) Restore through communication.
9) To minimize traffic detention.
10)To establish a real cause of accident.
11)To assess the impact of an accident.
12)To have a perfect proposal immediately helper who can help injuries.
13)To fix responsibility.
STEPS OF CONDUCTING ACCIDENT INVESTIGATION.
 Secure the scene of an accident. For example by Using flags and
fix detonators.
 Plan investigation.
 Collect data.
 Analysis of data.
 Active correcting.
 Do experiment if possible and necessary
 Write report.
WHY SECURING THE ACCIDENT SCENE?
We do secure the Accident Scene in Order to:
 Prevent another accident from happening.
 Help to prevent injuries and the health and control costs from
accidental loss.
 Protect well being of their employees and passengers.
WHY NEED TO TAKE NOTE OF RECORDS OF SITE OF ACCIDENT.
 To get the actual statistics of accidents occurred.
 Help to prevent injuries and ill health, and control costs from accident loss.
 Provide a reminder of possible hazards and accidents that occur;
 Provides a way to monitor potential problems and root causes as they recur.
 To increase the likelihood that repeating failures will be noticed and
corrected before they cause into more serious incidences.

FORMAL INQUIRY REPORT & SUMMARIES.
The formal Inquiry report and summaries means the official questions
asked to get information about the accident that has occurred, to need
to know the actual situation before and after the occurrence of an
accident. Usually asked to the Train Crews and witness of the
accident.
This formal Inquiry reports and summaries takes us direct to the Joint
Inquiry.
JOINT INQUIRY.
Is an investigation committee involving different staff/employee with
different professional and witness.
In TRC joint inquiry is attended by different departments which are ;
1. Traffic department.
2. Civil department.
3. Mechanical department.
4. Safety department.
5. Telecommunication department.
Levels of Joint inquiry;
a)Inspectorate level.
b)District level.
c)Head Quarter level (HQ).
A)Inspectorate level.
This is the level of inquiry which deal with the offence or accident
which required inspection. Especially to the yard.
On this level the staffs concerned are;
I. DTI (District Traffic Inspector) - Chair person.
II.DPWI (District Permanent Way Inspector)
III.DCWI (District Carriage and Wagon Inspector)
IV.DSTEI (District Signal Telecom Engineer Inspector)
V.STI (Section Traffic Inspector)
B) District level.
This is the one of the levels of enquiry which is done within particular
district. It deal with all cases occur within the specific district.
On this level the staffs concerned are;
I. DTM (District Traffic Manager) - Chair person.
II.DME
III.DTI
IV.STI
C) Head Quarter level.
This is the highest level of the joint inquiry which is the chief
Head Quarter Control.
An Example of of a Formal inquiry report & summaries which
involves the accident at Tabora station on 21/04/2023 morning time.
THE DISTRICT JOINT INQUIRY HELD AT CONTROL OFFICE
ON 21/04/2023 DISCUSSING ON THE ACCIDENT OF TRAIN
BD16 WITH LOCOMOTIVE NUMBER 9019 ON 21/04/2023 WITH
LOCO DRIVER LAZARO AND TRAIN GUARD MOSES,
INVOLVING BREAK OF COUPLING OF WAGON NUMBER

What is needed is to call for the information from the people who
have attended the accident, the situation of accident found and the
immediate actions taken after them arriving at the site of accident.
The ones who will be interrogated are the DCE, DCWI and the
train crews like the Loco pilot and the train guard and other
witness like the Yard Master and Points man.
The main objectives of the Formal Inquiry Report & Summaries
are;
1.To determine the actual cause of an accident.
2.To prevent the accident that resembles such accident.

Accident.
