Acute periostitis

Definition
Periostitis is a conditions that results in
inflammation of the band of tissue that surrounds
your bones known as the periosteum.

•This condition typically affects people who


repetitively:
1-Jump.
2-Run.
3-Lift heavy weight.
Type Odontogenic Periostitis
1-Acute
-Serous.
-Purulent (Acute Dentoalveolar Abscess ).

2-Chronic
-Ordinary or simple.
-Ossifying or hyperplastic ( Proliferative
ossification ).
-Rarefying ( Resorption with following
replacement osseous tissue on fibrous tissue).
Symptoms of acute Periostitis
1-Intense pain.
2-Difficulty bearing weight on the affected
limb pus formation.
3-Fever.
4-Chills.
5-Swelling of the tissue surrounding the
bone.
Causes of acute periostitis
•The most common causes of acute periostitis of the jaws are:
1-Exacerbation of chronic periodontitis.
2-Suppuration of odontogenic cysts.
3-Alveolitis.
4-Difficult eruption of 3 molars.
5-Suppurating odontomas.
6-Impacted semi-impacted teeth.
7-Complex tooth extraction.
8-As result of poor quality endodontic treatment.
Complaints in acute serous periostitis
•Acute serous periostitis is observed in
the first 2-3 days of the disease.Patients
note:

-Intense pain that is felt constantly.


-Swelling of the cheeks, lips.
Clinically
-A damaged tooth (either not treated or a filling
of poor quality).
-Periodontal pocket.
-Smoothness of the transition fold.
-Pain when percussing the “causal tooth”.
-When cutting the mucosa and periosteum we
can say that the periosteum is infiltrated and
hyperemic.
-If left untreated serous periostitis turns into
purulent periostitis.
Complaints with purulent periostitis
-Intense pain.

-Soft tissue swelling.

-Violation of general condition (weakness, fatigue,


elevated body temperature).

-Pain when touched.

-Pain when percussing the “causal tooth”.


Acute purulent periostitis

Is differentiated with:

-Acute and exacerbated chronic apical Periodontitis.

-Acute osteomyelitis.

-Abscesses and phlegmons.

-Acute sialadenitis.
DIAGNOSIS

-X-ray which may reveal fractures or signs of


damage due to infection.

-MRI scan which can provide a detailed look at


the bone and the surrounding soft tissue.

-Bone scans to determine if an infection is


present.
Treatment

•Acute Serous Periostitis


-Drilling or removing the causitive tooth.
-Anti-inflammatory medication.
-Physiotherapy.

•Acute purulent periostitis


-Intraoral incision and dranage
of subperiosteal abscess.
-Opening or removing the
causitive tooth.
-Anti-inflammatory medication.
