summary of the incident

While transporting ore material from SH 2 stockpile to Mesto crusher pad, ADT
number 18 operator stopped at stop sign, saw fire behind the cabin, got out of the
cab to use portable fire extinguisher. He was unable to manage fire; so, the control
room was alerted. SEMOS ERT assisted with fire truck and PW water bowser to
manage fire. This fire resulted in damages on the truck’s rear tires and hoses. No
injury sustained.

ANALYSE OF SUMMARY

Direct causes:
The basic cause of the incident was Broken propeller shaft cross bearing, which
as a result damaged the parking brake air hose and the hydraulic hoses around.
Since the parking brake is spring applied air release, it automatically applied in the
absence of air pressure, resulting into friction, which caused the fire in the
presence of leaking hydraulic oil.

Summary of contributing factors:


1. No automatic fire suppression system installed on equipment.
2. Only one 09 kg fire extinguisher bottle found on equipment.

KEY LEARNINGS

 To prevent normal wear and tear, at each shift change


service time we need to reinforce control of Propeller shaft
cross bearing of equipment.
 Increase the number of fire extinguishers on all equipment
by two of 09 kg fire extinguisher bottles before the
installation of automatic fire suppression system on all
equipment.
 SFTP must install automatic or semi-automatic fire
suppression system on the equipment.
 The time of reaction to manage the fire was long as we
could have quickly used onboard PW water truck.
