MTS SFTP ADT 18 Fire Incident Presentation ICAM DRAFT Report of 22 ND July 2023 - V2
While transporting ore material from SH 2 stockpile to Mesto crusher pad, ADT
number 18 operator stopped at stop sign, saw fire behind the cabin, got out of the
cab to use portable fire extinguisher. He was unable to manage fire; so, the control
room was alerted. SEMOS ERT assisted with fire truck and PW water bowser to
manage fire. This fire resulted in damages on the truck’s rear tires and hoses. No
injury sustained.

ANALYSE OF SUMMARY
Direct causes:
The basic cause of the incident was Broken propeller shaft cross bearing, which
as a result damaged the parking brake air hose and the hydraulic hoses around.
Since the parking brake is spring applied air release, it automatically applied in the
absence of air pressure, resulting into friction, which caused the fire in the
presence of leaking hydraulic oil.

Because the propeller shaft cross bearing got broke and removed from its place.
