CLEAN FUEL PROJECT Initial incident report: NEAR MISS (Drop Object)

Date and Time : 26-FEB-2024 – 15:30 Hrs.

Potential classification : DROP OBJECT
Incident Summary: At 15:30 AM, a scaffolding plank fell from approximately 10 meters height from the scaffolding
platform. The CKC piping team was installing small bore pipe when the incident occurred. The
scaffolding planks slipped during adjusting the gap and fell on the ground. Fortunately, there were no
injuries involved in the incident.

Observer: TOP & Subcontractor Construction Team
Cause of incident: Drop Object

Immediate action: Activity Suspended, Area Barricaded. Further Investigation in progress.
