Registration Period: Voucher/Reference Number should be registered within three (3) months from
the date of released. Otherwise, subject to cancellation of voucher or reference number.
H. Effectivity of Coverage
1. Effective Date

Effective date shall be based on the the E-card/reference number/voucher

registration date of the member. The date shall be monitored thru the shared file via
Google sheets across all Primary Care Center and Maxicare Tower.

2. Validity of Coverage

Coverage is one (1) year from effective date
