Walk Through
Cover Page
• Fill all fields on the cover page
appropriately
Instructions (1)
• Keep the register in a locked room /cabinet to prevent
unauthorized access.
• Register works together with Register B
• Always use both registers side by side
• Circle selected options. Do not use ticks, cross, underline,
etc.
• When page is full, count circled options and write the
numbers into the Page Summary at the bottom of each page
• Summary-boxes should not be left empty
• Start a new page for each new month

• Fill the Monthly Index Testing Report by adding


each box of the Page Summary for the respective
quarter
• The Column numbers at the very bottom of the
page link with the numbered boxes on the
Reporting Form
• Record all clients who were offered index testing
(IT) services. Even if no eligible contacts were
found

1. Index Client ID
• Assign a unique running number for each index client in registration
sequence.
• Do not re-start counting at the beginning of the financial year (e.g.1st July).
• Continue with running numbers indefinitely

2. Date of recording the index client
• Write the date that the index client was interviewed.
• Use the same format as in the contact register

3. Circle the correct entry point for the index client:
• HTS: Newly diagnosed positive or previously known positives who are
not on
• ART: Identified at ART clinic, for example clients on ART with
unsuppressed viral load

4. HTS Client Serial No / ART Patient ID
• Write the HTS serial number from HTS register
• Or write HTS link ID from confirmatory ScanForm register
• Or write the ART number from ART register

6. Age and Sex
• Age in years for adults, age in months for young children. Example: Write
27 for 27 years, write 16M for 16 months
• Sex of the index client

• New HIV Positive: Newly diagnosed index client from HTS

Known HIV positive and not on ART: Confirmed HIV positive client but
not on ART (possibly from confirmatory HIV testing)

On ART: All ART patients should be interviewed for potential contacts once
per year. Ideally this should be done when returning viral load results to
allow prioritizing patients with unsuppressed viral load for contact tracing

8. Consented to discuss contacts
Circle if the index client has provided informed consent to discuss current
or past sexual partners and for potential partner notification and contacts
tracing
• What Happens if they don’t consent ?
• Remember the legal age for consent!

9. Number of Contacts reported AND their HIV /ART status
• Write the number of contacts provided by the index client under each
category

9. Index Client is 15+ or Sexually Active Minor
Ask all adult index clients for the number of:
Sexual partners: All current or recent partners (from within the last 12
months)

Children 0-15 years: Include all children for HIV positive mothers.
• Include children of HIV positive fathers only if their biological mother is
known HIV positive or of unknown HIV status, for example if she is
deceased

9. Index is child 0-14 years
Ask all index children for the number of:
o Biological parents who are alive and may be traced for HTS.
o Always list the mother unless she is deceased.
o Include the father if the mother is HIV positive or deceased

These contacts don’t need to be followed :


Recent HIV negative: The contact tested HIV negative within the last 12
months
HIV+ on ART: The contact is known HIV positive and on ART

These contacts need follow-up. Write their details into the contact
register:
► Unknown HIV status: The index client is not aware of the contact’s HIV
status. Needs HTS.
► HIV+ not on ART: The contact is known HIV positive but not on ART.
Need confirmatory testing and linkage to ART clinic

Note:
The sum of contacts with unknown HIV status + HIV positive contacts not on
ART
from all 3 groups of columns must equal the number of contacts listed in
Register B

Index Scenario 1

• John aged 18 is a newly diagnosed on 14/07/21


• He has consented to provide contacts to be
followed-up as follows
• Jane , 26 years, Unknown HIV status
• Martha , 15 years, Tested negative 11 months
ago
• Jessica, 19 yrs, Tested positive but was not on
ART
Index Scenario 2

• On 13/07/21 ,James, aged 38 is newly


diagnosed with HIV
• He has not consented to provide
contacts to be followed-up
Highlight that there are no details for the number of contacts
reported and their HIV /ART status because the index did
not provide consent
Index Scenario 3

• Ndaziona is a newly diagnosed girl aged 15


and is sexually active .She has consented to
provide contacts to be followed-up:
• Kezie, 19 yrs old , unknown HIV status
• Jazman 12 yrs old , tested neg. 11 months
ago
• Mayeso 22 yrs old , tested positive 2 yrs
ago and is on ART
