and activities for their effective and efficient implementation
at the school level, in partnership with various stakeholders
● Focus on DepEd’s major programs:
1. School-Based Feeding Program (SBFP);
2. National Drug Education Program (NDEP);
3. Adolescent Reproductive Health (ARH) Program;
4. Water, Sanitation, and Hygiene (WASH) in Schools (WinS)
5. Medical, Dental and Nursing Services

1. Conduct of health examination
a. Health appraisal/ assessment – Health card of learners
b. first aid, referrals of learners to specialists
c. athletes and coaches in sports events
d. vision screening (K, G1, G4, G7, G10) - Snellen chart
e. color vision testing (G10) - Ishihara test
f. hearing screening - Tuning fork
2. Inventory of medical supplies
3. Issuance of permission slips to pupils to be signed by parents/guardians
4. Conduct of post conference to school administrators to discuss observations
and findings of cases and recommendations
5. Integration of health service delivery in the SIP

6. Conduct of the School-based Immunization with LGU
a. Measles, Rubella, Td for G1, G7
b. HPV for G4 female learners
7. WIFA for Grades 7-12 female learners

9. Discussion of health concepts in the Curriculum

RECREATION AND RELAXATION ACTIVITIES FOR SENIOR CITIZEN AND PERSON WITH
DISSABILITIES FOR TEACHING AND NON-TEACHING PERSONNEL OF SDO CAVITE CITY
ANNUAL PHYSICAL AND DENTAL EXAMINATION OF TEACHING AND NON-TEACHING PERSONNEL FOR SY 2023-2024

12. Enlistment of health personnel to augment the health manpower of DepEd
13. Prepare schedule of school visits/deployment plan (monthly and yearly)
14. Mobilize the teachers and community/parents to support the activity
15. Provision of mental health and psychosocial support in collaboration with
Guidance counselors
16. Psychological First Aid in collaboration with DRRM
17. Ensure compliance with data privacy act
18. Ensure proper safe keeping of the health records
19. Submit reports
20. Conduct advocacies for health programs
● submit a medical certificate (3 months validity) from their
physician of choice until August 30, 2024
● participate in the physical and dental examination conducted by the
SHNU wherein they must complete the laboratory examinations on
their preferred laboratories and submit the results
● free chest x-ray procedure provided by the Cavite Positive Action Group
(CPAG)
● Chest x-ray schedule must be strictly 150 pax per day therefore school
health program coordinators along with the school principals must
submit a master list of personnel who will voluntarily undergo the free
chest x-ray and physical examination (Laboratories are excluded)

completed the day before the face-to-face consultation schedule and
must be presented in hard copy during the APE

● Others as the office may see fit
We are proposing to have a group HMO card for the
personnel
