ANNOTATION
I've attached the COT rating sheet for Q1–Q4 for my first objective. A score of 7
under indication 1 shows that I have successfully met the objectives and that I
am able to apply knowledge across different curricula in teaching areas. I
carefully develop a daily lesson plan, making sure that all performance and
learning requirements are satisfied, because I have a firm grasp of the course
material in areas dedicated to teaching.
COT 1
COT 3 COT 4
ANNOTATION
Attached is the COT rating sheet covering Q1 through Q4, which is my second
objective. Having received a score of seven under indicator two, I can confidently
say that I have fulfilled the requirements and can effectively include reading and
numeracy teaching and learning activities. As an example, during my lesson during
observation, I had my students read the provided text regarding cyberbullying and
they were to use a bar graph to determine the total percentage of people who
experienced cyberbullying between 2010 and 2013.

COT 1
COT 3 COT 4
ANNOTATION
The COT rating sheet for Q1 through Q4 is attached as my third objective. With a score of
6 for my COT 1 and 7 for my COT 2 to 4, indication 3 stresses the successful use of
different teaching methodologies to encourage critical thinking, creative thinking, and
higher-order cognitive skills. I created a lesson plan that contains comprehension
questions, performance assignments with instructions and rubrics, and summative exam
assessments that cover both lower and higher-order thinking skills. Furthermore, I focus
implementing real-life applications of the lesson to enhance the students' learning
experience.

environments to enhance
learning through the
consistent implementation
of policies, guidelines and
procedures.
COT 3
ANNOTATION
The attached COT rating form for indicator 5 with the ratings 6 for COT 1 and 7
for COT 3 shows that I successfully achieved the objectives by establishing a
safe and secure learning environment for my students. I make sure they are
aware of my classroom procedures. By fostering a pleasant classroom
atmosphere, I can provide students with the safety and support they require for
academic, personal, and professional growth. Furthermore, by regularly
implementing well-defined regulations, norms, and procedures, a safe and
secure environment is produced, which improves students' overall learning
experiences.
COT 3
ANNOTATION
The attached COT rating form for indicator 6 with ratings of 7 highlights my
success in maintaining an educational environment that consistently supports and
promotes fairness, respect, and care. By doing so, it aims to create a space where
students are motivated and encouraged to engage in their learning, leading to
better educational outcomes.
COT 4
ANNOTATION
The attached COT rating sheet for indicator 7 demonstrates how I successfully established
a learner-centered culture during the synchronous teaching observations of COTs 1–4,
resulting in consistent 7 ratings. This success is due to my ability to use instructional tactics
that are sensitive to my learners' different language, cultural, socioeconomic, and religious
backgrounds. I have used a variety of inclusive teaching strategies to fit each learner's
individual requirements and traits. By doing so, I've created an inclusive and supportive
learning atmosphere in which all students may grow and fully participate in their studies.
COT 4
ANNOTATION
The attached COT rating sheet for indicator 8 indicates how I successfully changed and
implemented culturally appropriate teaching strategies to meet the requirements of
indigenous learners during the synchronous teaching observations of COTs 1-4, resulting in
consistent 7 ratings. This highlights the significance of adapting and implementing culturally
relevant teaching practices to fulfill the educational demands of indigenous learners. This
includes comprehending and incorporating indigenous cultural backgrounds into teaching
methods, establishing an inclusive and supportive learning environment for indigenous
students, and addressing their special educational problems and requirements.

Objective 9
COT 4
ANNOTATION
I have included the tenth objective—which is about applying tactics for timely, accurate, and
constructive feedback to improve learner performance—into my teaching. I have provided
the COT rating form from my in-person synchronous teaching observations as proof of my
accomplishment of this goal to bolster my assertions further. I scored a 7 on indicator 10,
demonstrating my ability to consistently use practical tactics that help my students receive
insightful feedback. This is providing feedback on time, making sure it's accurate, and
making it constructive to encourage advancement in their work. By doing this, I have
fostered an atmosphere where students are encouraged to keep growing and developing,
which eventually helps them succeed academically.
Utilized assessment
data to inform the
modification of
teaching and learning
practices and
programs.
ANNOTATION
Using the assessment data, I have taken into account the frequency of accurate responses to
address the subjects' least learned competencies. The data above shows that, out of the thirty
students in my class, the least learned competencies in the subject of empowerment were
obtained by 3.33%, UCSP by 46.67%, and PAGBASA by 66.67%. This implies that the
teacher actively gathered, examined, and interpreted a range of assessment data—tests,
quizzes, assignments, and observations—in order to evaluate students' performance and
comprehension. It also emphasizes the significance of applying data-driven decision-making
to continuously enhance both specific teaching strategies and more comprehensive
educational initiatives, guaranteeing that they successfully support students' learning and
development.
This is my assessment
where almost all of my
students got the highest
score.
ANNOTATION
I have attached my class record and DLP to this objective. As you can see, almost
all of my students received the top results on my written test, according to the
data that was supplied. It guarantees that choices are supported by facts and
accurately represent the needs and abilities of students.

ANNOTATION
Active involvement in the educational process is facilitated by building relationships with
parents, guardians, stakeholders, and the larger school community. We may foster a
collaborative environment that aids in the growth and development of our students by
fostering this relationship. The narrative report, attendance data, minutes from the
meeting, giving of certificates of appreciation from the various stakeholders, and images
from a homeroom-parent-teacher conference are all included to highlight the relevance
of this activity. Our ability to better comprehend the opinions and concerns of parents
and the community will enable us to address and adapt our teaching strategies.
